In order for this form to work, you need to set the email address the message will be sent to:

1. Edit your contact page in the wordpress admin
2. Go down to the "custom fields" section and click "enter new"
3. Enter "my_email" in the "name" field
4. Put your email in the value field
5. Click "add custom field"
6. Save your page
